Referrals and Collaborative Care – Coordinated Support Within and Beyond Your Team

Our Approach

Referrals & Collaboration

Good counseling rarely happens in complete isolation from other professionals. A client working with a counselor may also be seeing a psychiatrist, a school welfare officer, a social worker, or a paediatrician. Care decisions made by one professional affect the others. When these relationships are coordinated, outcomes improve. When they are fragmented — when each professional works from their own siloed records without visibility into what others are doing — important information gets lost and interventions can conflict.

REVYV Clinik treats referrals and collaboration as first-class functions of the counseling platform, not afterthoughts. Internal referrals between team members, external referrals to allied professionals, and multi-disciplinary coordination across organizations are all managed and tracked within the client record.

Referral Management

How REVYV Clinik Handles Referrals

Internal Referrals

Refer a client from one counselor to another within the same team — with the full case record transferred to the new counselor at the point of referral. No information loss, no handover gaps. The receiving counselor sees everything the referring counselor documented.

External Referrals

Refer clients to healthcare providers, community services, specialist practitioners, or other external organizations. The referral is logged against the client record with the referred-to professional, purpose, urgency, and status — enabling follow-up and outcome tracking.

Referral Tracking

Every referral has a status — pending, accepted, declined, completed — that updates over time. Counselors know whether the referral resulted in the client receiving the referred support. Outcomes are documented as part of the case record.

Incoming Referrals

Cases arriving from external referrers — healthcare professionals, schools, community organizations, social workers — are recorded with the referral source, reason, and urgency. Assignment to the right counselor follows the same structured intake process as all new cases.

Collaboration

Collaborative Care Across Professionals

When multiple professionals are involved in a client's care, coordination depends on information sharing. REVYV Clinik supports collaborative care through role-based access — enabling the right professionals to access the parts of the client record relevant to their role, without exposing information they should not see.

A supervisor can review a counselor's session notes and care plan without seeing family communication the counselor has marked confidential. An allied professional brought into a collaborative care arrangement can access shared documents and care plan goals without having access to the full clinical record. Role-based access is not a technical detail — it is what makes genuine multidisciplinary collaboration possible in a platform that also protects client privacy.
