    The counselor enters session information — key topics covered, interventions used, client responses, and any concerns or follow-up actions — in a structured input area or through brief notes.

  • AI Generates a Draft

    REVYV Clinik's AI organizes the input into the selected note format (DAP, SOAP, BIRP, GIRP, or progress note), producing a structured draft that reflects the session content. The AI can also highlight possible follow-up items and identify concerns mentioned during the session.

  • Counselor Reviews and Edits

    The draft is presented to the counselor for review. The counselor can edit any section, correct inaccuracies, add clinical observations, and adjust the tone or emphasis of the note before approving it.

  • Approval Creates the Record

    Once approved, the finalized note becomes part of the permanent client record — linked to the session, the care plan, and the broader case file. No AI-generated content enters the record without the counselor's explicit approval.
