How Assessments Work in REVYV Clinik

Create and Assign

Counselors can create assessments from available templates and assign them to a client at intake, during care, or when a specific concern arises. Assessments can be completed in-session or sent for the client to complete independently.

Results in the Client Record

Completed assessment results are automatically stored as part of the client's case record, alongside session notes, care plans, and referral history. Counselors do not need to transfer or re-enter results.

Trend Comparison Over Time

When the same assessment is administered at multiple points during care, counselors can compare results to identify whether the client's responses have shifted — improving, stable, or declining in a specific area.

Counselor Review and Interpretation

Assessment results support counselor judgment — they do not replace it. Counselors remain responsible for reviewing results, interpreting client needs, and determining the appropriate course of care.
